Instructions:

1. Page 1-3 must be completed by all student athletes in grades 7-12.

2. Page 4 must be completed by all incoming 7th, 9th and 11th grade student athletes or students answering "yes" to questions 1, 2, 5,  7, 11 or 16 on the UIL medical history form.

3. High school students should turn in completed forms to the athletic trainers, not their coach.

4. Middle school students should turn in forms to their coach.
