Immunization Program Information

The Brazosport Independent School
District Board of Trustees recommends that each child have a
physical examination immediately before entering school for the
first time, before entering intermediate school and before entering
high school.
Students entering any school of the
Brazosport Independent School District for the first time shall have
been immunized to comply with Texas State Law. Previously enrolled students should have received notification
regarding updating of immunizations. For further information, parents
should contact their physician, the health department, or their school
nurse.
Evidence of compliance with the immunization
requirements must be validated by a doctor who is registered and
licensed or a public health clinic.
Required Immunizations
The law requires that students be fully
vaccinated against the specified diseases. A student may be enrolled
provisionally if the student has an immunization record that indicates
the student has received at least one dose of each specified
age-appropriate vaccine required by this rule. To remain enrolled,
the student must complete the required subsequent doses in each
vaccine series on schedule and as rapidly as is medically feasible and
provide acceptable evidence of vaccination to the school. A school
nurse or school administrator shall review the immunization status of
a provisionally enrolled student every 30 days to ensure continued
compliance in completing the required doses of vaccination. If, at
the end of the 30-day period, a student has not received a subsequent
dose of vaccine, the student is not in compliance and the school shall
exclude the student from school attendance until the required dose is
administered.
Transfer students to the district shall
be required to show evidence of compliance with these policies.
Students who need to have inoculation
temporarily postponed because of illness or other medical reasons must
present a written statement from the family physician.

In addition, please be aware of the

Tuberculosis - Proof of a
tuberculin skin test is required for all students entering Brazosport I.S.D.
from Mexico, South or Central America, Africa, Southeast Asia, or
India. |

Bacterial Meningitis – is not
a required vaccine, however, it is recommended for young children.

Exemptions
Students requesting exemption from any
of the immunization vaccine requirements must present to the admitting
school official one of the following:

Medical - An affidavit signed by a doctor who
is registered and licensed under the Medical Practice Act of Texas
and stating that in the doctor's opinion the immunization required
would be injurious to the health and well-being of the student or
any member of his family or household. The law does not allow parents/guardians to elect
an exemption simply because of inconvenience (a record is lost or
incomplete and it is too much trouble to go to a physician or clinic
to correct the problem). This must be updated yearly. |

Reasons of Conscience-The immunization
requirements now include an exemption from immunizations for reasons
of conscience, including a religious belief. This change was a
result of House Bill 2292 passed by the 78th Legislature and became
effective on September 1, 2003. To claim exclusion for reasons of
conscience, including a religious belief, the child’s parent or
legal guardian must present a signed affidavit form to the school.
The affidavit will be valid for a two-year period. This
exemption does not apply in time of emergency or epidemic declared
by the Commission of Health. |
Written request must be submitted
through the U.S. Postal Service, commercial carrier, fax, or by
hand-delivery to:

Affidavit form requests will be
processed and mailed within one week from the receipt of the request.
The letter must include the following information:

Full name of each child
for whom a form is requested (first, middle, and last);

Date of birth of each
child for whom a form is requested;

Parent or legal
guardian’s complete return mailing address, including zip code; and

 |
Number of forms needed
for each child (not to exceed five forms per child)

DSHS Recommended Immunization
Schedule for Children Ages 0–6 Years

The Brazoria County Health Dept. Clinic at 200 South
Hwy 288-B in Clute will be open Monday through Friday 9:00 –
11:30 am and 1:00 – 4:00 pm. Children must be accompanied by a
parent or guardian and bring a current immunization record with
them.
Effective September 1, 2005, the Texas Dept. of Health Services
required that children attending child-care facilities,
pre-kindergarten, Head Start, or Early Childhood programs receive
additional vaccinations for invasive Pneumococcal and Hepatitis A
disease (House Bill 1316). In BISD this affected 3 yr. olds
attending Early Childhood classes and 4 yr. olds attending Pre-K
classes. Students must have begun or completed the shots by Sept. 1,
2005 or they will be excluded. Please contact your physician, the
Brazoria County Health Dept, or your school nurse for more
information. |
