Brazosport ISD Parents and Guardians,

During the 89th Legislative Session, Senate Bill 12 was passed, requiring school districts to inform parents and guardians of their rights and to seek annual parental consent for health-related services, as well as the right to deny counseling services.

For BISD to continue to offer health-related services for the 2025–26 school year, parents must complete a form using the online registration link provided below. Additionally, an acknowledgment of receipt for counseling services is requested.

Forms will be available beginning August 30, 2025, and must be submitted by September 12, 2025.
We will continue to support your students’ health-related services as in previous years in the interim.

• Login to your Parent Self-Serve Account: Parent Self-Serve Login (Registration)
• Go to the 'Registration' TAB
  
  
• Scroll to the bottom and click on 'Continue to Online Registration'
  
• Choose your student and then click 'Next' five times until you get to the 'Student Forms' tab.
  
• On this screen, scroll all the way to the bottom and complete both forms (all other forms on this screen were completed during the online registration/enrollment process, so there is no need to change any of them).
