Absence Notes

•  A student will not be counted absent for an appointment with a doctor, dentist, or other health care professional if the student attends school at any time on the day of the appointment and provides appropriate documentation.
• To submit an absence note, click the name of your student's campus below and complete the online Absence Note Form. The form will be sent directly to the appropriate campus for processing.
• Please include the following information when completing the form:
  • Student's name, ID number and grade
  • Date(s) of the absence(s)
  • Reason for the absence(s)
  • Parent/guardian's name
  • Parent/guardian's daytime telephone number or contact number
• Parents are required to submit absence notes within 48 hours (two business days) of the student's return to campus. Absence notes received after this timeframe will be considered unexcused unless otherwise permitted by district policy.
• A principal may require a physician's verification of an illness at any time.
• If you have questions or concerns regarding your student's attendance, please use the campus attendance email address listed below. The attendance email should be used for questions or communication with the campus, while absence notes should be submitted through the online form.
• A note regarding a student's absence signed by the student, even with the parent's permission, will be considered a forgery, and the student will be subject to disciplinary action.
